MAUI COUNTY FEDERAL CREDIT UNION MEMBER SURVEY SWEEPSTAKES OFFICIAL RULES

1. Sponsoring Organization This sweepstakes is sponsored by Maui County Federal Credit Union ("Credit Union"), located at 224 Kehalani Village Drive, Wailuku, HI 96793.
2. Eligibility The sweepstakes is open to all Maui County Federal Credit Union members who complete the member survey in its entirety by the entry deadline. Employees of Maui County Federal Credit Union are not eligible to participate. Void where prohibited by law.
3. Survey and Entry Period The survey will be distributed to eligible members beginning May 11, 2026. The entry deadline is June 12, 2026, at 11:59 PM HST. No entries will be accepted after this date and time. Completion of the survey and entry form constitutes the member's sole entry into the sweepstakes. Limit one (1) entry per member.
4. Prize One (1) winner will be selected to receive a $250 Visa gift card. The odds of winning depend on the total number of eligible entries received. No cash or other substitution of the prize is permitted, except at the sole discretion of the Credit Union. The prize is non-transferable. The winner is solely responsible for any and all federal, state, and local taxes applicable to the prize. The Credit Union will comply with all applicable tax reporting requirements, including the issuance of an IRS Form 1099 if required by law.
5. Winner Selection One (1) winner will be selected by random drawing from all eligible entries on or before July 12, 2026 (within 30 days of the survey closing date). The drawing will be conducted by a representative of Maui County Federal Credit Union. The decision of the Credit Union is final.
6. Winner Notification The winner will be notified by email using the contact information on file with the Credit Union. The winner must respond within 14 days of notification to claim the prize. Failure to respond within this timeframe may result in forfeiture of the prize and selection of an alternate winner.
7. Privacy Member information collected through the survey will be used solely for the purposes of improving Credit Union services and administering this sweepstakes. Survey responses are confidential and will be handled in accordance with the Credit Union's privacy policy.
8. General Conditions The Credit Union reserves the right to cancel, suspend, or modify the sweepstakes if fraud, technical failures, or any other factor impairs the integrity of the giveaway, as determined by the Credit Union in its sole discretion. By participating, entrants agree to be bound by these Official Rules and the decisions of the Credit Union.
9. Winner Announcement The winner's name may be announced on the Credit Union's website and/or social media channels following notification and confirmation. If you do not wish to have your name publicly announced, please notify the Credit Union at the time of winner notification.
10. Governing Law This sweepstakes is subject to all applicable federal, state, and local laws and regulations and is governed by the laws of the State of Hawaii.
